Check the IDE/CF adapter, if the underside has solder pads then it is likely to be configured for a master and slave on that card, and anything else you add to the bus will conflict with the slave part of the IDE/CF adapter. If it has solder pads, even though nothing is soldered to it, the device is still configured with an active slave. Tip: the Chinese IDE/CF adapters generally DO have empty solder pads underneath and ARE configured for master/slave.
I got caught out on this, pulled my hair out for some time. I then found a CF adapter that was purely master only, and BOOM she worked
