In my photo of the card above you can see it for yourself, ICS249 - 304. According to your table that seems correct?

I gave you a list of the connections between the DRAM chips and the TC6058 on atari-forum.com. Have you checked this ...?
Artur write that the OGFA GAL works fine with his cards, he use only the TC6100 chip. So I thing it has only to do with the Address Signals A20, A21 and A22 at the TC6058 chip ...? I test a T0 Nova Driver card with TC6058 chip with the Nova VME Adapter and the OGFA GAL ...
On the Chip TC6058 ISA ET4000AX card there are A20,21,22 and A23 all at GND but I not know about the ISA Connector Pads A20,21,22 and A23 ...?

In the circuit diagram for the TC6058, A20 to A23 are connected to GND on the chip and on the ISA bus, LA20 to A23 are open and not connected, as are LA17 to LA19.
The PCB Layout is for the TC6100 chip. Pin37 is A16, A17 connect together with A20, A18 connect together with A21, A19 connect together with A22, A23 at GND
I tested a ISA ET4000 card with the TC6058 chip in a Nova VME to ISA Adapter with the OGFA GAL and all works fine.
