CRT Monitor repairs service database

As there are less and less people repairing CRTs nowadays, perhaps it would be a good idea to compile a list of individuals or companies, who still offer this service.
Perhaps we should also classify by region covered, as I dont know about anyone else, but I would be very reluctant to post one...and thats if you can physically lift it! In which case, maybe callout area should be included.
So details should perhaps be:

MODELS COVERED:
LOCATION:
CALLOUT AVAILABLE: Y/N MILAGE:
And perhaps one of the good moderators here could sticky this too.
